Swamiji, my father died due to cardiac arrest. It was totally unexpected. Me, my mum and sister are experiencing extreme grief. My question swamiji, how true is that if the wishes of the dead are unfulfilled at time of death then the soul again takes birth in the same family? Does that really happen?

Yes, please tell your mother and sister, instead of thinking and talking about the death or absence of your dad, ask them to pray sincerely that departed soul will be born again in your family. You can see a child who is having exact character, nature and behaviour of your dad.
See, thinking and crying on absence will invite permanent absence and however, if they think about presence in different form it will become so true.
